It is largely preventable when evidence-based catheter practices are followed.
Catheterization itself is not inherently dangerous; unnecessary catheterization, traumatic insertion, poor aseptic technique, prolonged catheter dwell time, and inadequate maintenance are the major risk factors.

Balloon Volume

Aseptic Field & Cleansing

Gentle Advancement
Advance slowly. In males, advance completely to the 'Y-hub' regardless of when urine returns. If resistance is met, STOP immediately.
Confirmation & Inflation
Only inflate balloon once urine is clearly visible and draining freely. Inflating in the urethra causes catastrophic injury. Secure to thigh.

Anatomical Obstacles
Meatal Recession
Estrogen deficiency in post-menopausal women causes the meatus to retract inward or fuse. Never use "blind" force probing the anterior vaginal wall.
Pelvic Organ Prolapse
Cystoceles can severely distort the urethral path. Gentle manual reduction of the prolapse may be required to clear the instrument pathway safely.
Obesity & Retraction
In cases of severe adiposity, a second operator is mandatory to maintain labial retraction, ensuring the sterile field is not compromised during visualization.

The Vaginal Marker Rule: If the catheter mistakenly enters the vagina, leave it there. It serves as a visual guide and physical block to avoid re-insertion in the same orifice while you fetch a new, sterile catheter.

Lateral Decubitus Positioning: For patients with severe arthritis or obesity where the frog-leg position is impossible, place the patient on their side with the upper leg flexed to improve visual access.

Coude Use in Females: Do not hesitate to use a Coude tip (pointing anteriorly) in females with high meatal recession to navigate the distorted angle.
Complications & The "Never" List
Chapter 10 & 11: Traumatic instrumentation is the primary driver of bacterial prostatitis and false passages. If resistance is met, seek expert intervention immediately.

Bacterial Prostatitis (Ch 10)
Traumatic instrumentation seeds meatal bacteria into the prostate. Watch for post-op fever, pelvic pain, or epididymo-orchitis in men.
Hematuria / Blood at Meatus
Indicates mucosal injury. Requires larger catheter (to tamponade) placed by a senior clinician or urologist.
Intra-Urethral Inflation
Causes severe pain and rupture. Deflate immediately. Urological consult mandatory to assess stricture risk.
